Anthem Inc is a health benefits company which offers a spectrum of network-based managed care plans to the large and small employer, individual, Medicaid and Medicare markets.

Anthem Inc is an Indiana corporation incorporated on July 17, 2001. It is a health benefits company serving approximately 38.6 medical members through its affiliated health plans. The Company offers network-based managed care plans to large and small employer, individual, Medicaid and Medicare markets. The Company operates in segments including Commercial and Specialty Business; Government Business; and Other. The Commercial and Specialty Business segment includes Local Group, National Accounts, Individual and Specialty businesses. Business units in the Commercial and Specialty Business segment offer fully-insured health products; provides managed care services to self-funded customers including claims processing, underwriting, stop loss insurance, actuarial services, provider network access, medical cost management, disease management, wellness programs and other administrative services; and provides specialty and other insurance products and services such as dental, vision, life and disability insurance benefits, radiology benefit management and analytics-driven personal health care guidance. The Company's Government Business segment includes Medicare and Medicaid businesses, National Government Services, or NGS, and services provided to the federal government in connection with FEP. Medicare business includes services such as Medicare Advantage, Medicare Part D, and Medicare Supplement. Medicaid business includes managed care alternatives through publicly funded health care programs, including Medicaid, Temporary Assistance for Needy Family programs, programs for seniors and people with disabilities, programs for long-term services and support, Children's Health Insurance Programs and ACA-related Medicaid expansion programs. NGS acts as a Medicare contractor for the federal government in several regions across the nation. The Company serves its members as the Blue Cross licensee for California and as the Blue Cross and Blue Shield, or BCBS, licensee for Colorado, Connecticut, Georgia, Indiana, Kentucky, Maine, Missouri (excluding 30 counties in the Kansas City area), Nevada, New Hampshire, New York (as BCBS in 10 New York City metropolitan and surrounding counties, and as Blue Cross or BCBS in selected upstate counties only), Ohio, Virginia (excluding the Northern Virginia suburbs of Washington, D.C.) and Wisconsin. The Company competes with other health benefits plans for the contracts with hospitals, physicians and other providers. The Company's operations are subject to state, federal and international regulation throughout the jurisdictions in which it does business.
