Delaware First Health (DFH) and the Centene Foundation have announced a new grant program focusing on improving social determinants of health across Delaware. The initiative aims to distribute a total of $500,000 over the next three years, with individual grants starting at $10,000 each. Applications are currently open and will be accepted until June 13, 2025.

This grant program is specifically targeted towards Delaware-based 501c3 organizations that cater to the Medicaid population. The objective is to enhance community well-being by addressing factors such as housing instability and transportation accessibility, although initiatives addressing other social health determinants will also be considered. To qualify, organizations must demonstrate measurable outcomes and impacts over time.

Award announcements are anticipated in July 2025, after all submissions have been thoroughly reviewed. The initiative will support projects that seek to alleviate any of the various social drivers of health, with a preference for those that address housing and transportation challenges.
